Introduction: Gastrointestinal cancers continue to be a significant cause of mortality and morbidity worldwide. Cancer immunotherapy is one of the new alternatives in cancer treatment. The aim is to use the patient's immune systems to fight cancer cells, overcoming the problem of specificity, which is a key difficulty in cancer treatment. Chemotherapy, radiation, and other therapies are now available. This review discusses recent advances and future directions of immunotherapy in patients with gastrointestinal malignancies. Methods: This study performed by using collected article in English were available on details of the main topic since ۲۰۰۰ to ۲۰۲۱ in Scopus, PubMed, Web of Science with keyword Gastrointestinal cancers, immunotherapy and treatments. Articles were selected on the basis of the exclusion criteria and after reviewing were included in the study. Results: According to studies, Immunotherapy is an effective and essential tool for this disease because the immune system can detect an almost innumerable number of antigens secondary to the biology of genetic recombination in B and T lymphocytes. The immune system is structured to differentiate itself from others. In addition, the immune system has mechanisms to inhibit its own responses, many of which are triggered by developing tumors. Understanding how cancer interacts with the immune system. Provides insights into mechanisms that can be used to inhibit anti-tumor immune response Conclusion: Although clinical malignancies may have bypassed endogenous antitumor immune responses, immunotherapy has the potential to increase responses to reduce tumor progression, and will likely have a growing role in it.
